  • Sept. 5, 2012: A final reminder that all fields on the Water Quality Data Entry Form for surface and groundwater monitoring reports are required to be completed, with the exception of the UTM Easting and UTM Northing codes when latitude and longitude coordinates are given. Failure to complete all required fields on surface and groundwater monitoring reports will result in a noncorrectable noncompliance under 405 KAR 16.110/18:110 (water monitoring).   • In an effort to simplify the submittal process of the SMP-61s and to expedite their review, the Division of Mine Reclamation and Enforcement will require submission of these documents in electronic format. All SMP-61s, if submitted to the regional offices, will be required to be submitted in electronic format beginning Aug. 16, 2012. Any questions should be directed to Raymond Hudson or Wes Jones at 502-564-2340.
 
  • Effective March 16, 2012, the Explosives and Blasting Branch will conduct a review of outstanding penalties for any company requesting a 40,000 pound waiver. Persons requesting a 40,000 pound waiver should be aware that any unpaid penalties will delay the processing of said requests. Citations that are in the appeal process will not incur any processing delays.
 


2011 Archive


  • SME80 for Minor Field Revisions is available for electronic submission and can be found on the Electronic Forms Web page. Completion of this form is the same as the application submission already available through the Division of Mine Permits. The package contains a permittee authorization that must be added as an attachment to the application.


  • SME21 and SME22 Electronic Submission - DMRE accepts electronic pond and hollowfill certifications through the Regional Office FTP sites. SME21 and SME22 forms are available on the DMRE Electronic Forms Web page. Information on how to submit to the FTP site can be found in the Electronic Submission Guide. The guide includes detailed instructions regarding the proper filing format and naming conventions for electronic certification submittals.
